Defensive Driving Training Requirements

State and University policy requires all employees who drive State or privately owned vehicles on State business attend a Defensive Driver Training (DDT) course once every four years and maintain an accident free good driving record. Classes are offered throughout the year. Call Environmental Health and Safety at X2124 for the next scheduled training session.

  • To be on the approved drivers list, departments must have Form INF 254 (Driver Record Information) on file in the University Police Department for all faculty, staff and student assistants who drive on State business. The INF 254 form can be obtained by clicking herePDF file or from the University Police Department.
  • Employees who use their private vehicles on State business must annually complete Form 261 (Authorization to Use Privately Owned Vehicle)PDF file. The original is kept in your department and a copy must go to the Business and Financial Affairs Travel Office (CP-300). The Form 261 can also be accessed from Informed Filler under the Travel section.
  • Employees who operate a 12-15 passenger van must attend a van safety training class offered every four years. Contact EHIS for the available class.
