Accident Investigation

Serious occupational injuries, illnesses or exposures to hazardous substances must be reported to EH&IS no later than 48 hours after they become known. EH&IS will contact Cal/OSHA as required by law. EH&IS, in conjunction with a representative from the injured employee's department, will investigate the circumstances of the incident to determine the cause.

EH&IS reserves the right to investigate any accident for which there was medical treatment or a correction to an unsafe condition is necessary. The purpose of an investigation is to find the cause of an accident, prevent future occurrences, not to fix blame.

A copy of each investigation report will be forwarded to the department of the injured party by EH&IS.
